Canadian Food Inspection Agency- Scrapie Quarantine Letter - Use this form when enrolling in the VSFCP. Sign this letter and send to your local CFIA District Veterinarian for verification.
Farm Site Plan - Use this farm when enrolling in the VSFCP and for the annual application for advancement when changes have been made to areas that contain animals.
Producer Contribution Form - Required for both enrollment and advancement application.
